Visual Field Recovery - Case Report of a Patient with Left Homonymous Hemianopsia Post-Stroke: Crimson Publishers


Optometrists specializing in Neuro-optometric rehabilitation have been working with physiatrists, neurologists and other rehabilitation specialist for decades, and have demonstrated that visual field recovery is possible for many patients beyond the expected time for recovery. The research proving that therapy can be effective is being done by neurologists who have demonstrated re-wiring of neural pathways with improved performance after repetitive visual-motor tasks and recovery of somatosensory and motor awareness after virtual reality (visual) stimulation [2].
In one UK study of 915 stroke patients 52% had visual field loss and 10% had no symptoms. Of the patients with visual field loss half had difficulty reading, blurry vision, diplopia, ocular motor and perceptual problems while half complained only of their peripheral vision loss. At follow up 15 patients (7.5%) had full recovery, 78 (39%) had improvement, and 104 (52%) had no recovery. Two patients (1%) had further decline of visual field. Patients with visual field loss had lower quality of life scores than stroke patients without visual impairment and patients with homonymous hemianopsia had worse overall prognosis for recovery [3].

Plasminogen is an enzyme that is linked to the dissolution of fibrin, and consequently thrombi. It plays an important role in the recruitment of macrophages during the inflammatory response. Plasminogen deficiency is a rare autosomal recessive disease, which may be heterozygotes asymptomatic or homozygou symptomatics. It can be correlated to appearance of low levels of plasminogen in children, whose both parents have reduced values [1].
The ligneous Conjunctivitis (LC) is the most common presentation of clinical syndromes associated with type I plasminogen deficiency [2,3]. It's a rare pathology and may be more prevalent in areas where consanguinity is more common [1,3]. Its diagnosis is based on clinical, histological and possibly positive family history. Clinical findings are usually: Chronic tearing, conjunctival hyperemia, pseudomembranous lesion in the conjunctiva and/or other mucosal sites of the body, possibly associated with colloid milium or obstructive congenital hydrocephalus. Such membranes are deeply adhered to the conjunctiva of the eyelid, with its free end. Removal tends to cause bleeding because the area is vascularized [4,5].

Diagnosing and Managing a Case with a Fourth Nerve Palsy and Papilledema_Crismson Publishers



A 14-year-old boy presented to the Hospital with new-onset vertical binocular diplopia. Medical history was notable for a recent cranial traumatism. On examination, visual acuity was 20/20 in each eye. Pupils were equal and reactive. Visual fields (VF) were full to confrontation. A slight right head-tilt was observed (Figure 1A), as well as a left hypertropia of 3 Diopters (DP) in primary gaze. Ductions and versions revealed mild over-elevation in adduction and under-depression in adduction on the left eye. Left side Bielschowski maneuver was positive (Figure 1B). Red filter in right eye-test showed an oblique diplopia, but the patient had no torsion on Maddox testing. However, dilated fundus examination revealed bilateral fundus torsion and papilledema (Figure 2). Optical coherence tomography (OCT) revealed an abnormal thickness of the retinal nerve fiber layer (RNFL) and a normal values of retinal ganglion cells (GC).

Natural killer (NK)/T-cell lymphoma is a rare primary non- Hodgkin lymphoma. It can be divided into nasal, non-nasal, and aggressive NK leukaemia/lymphoma subtypes [1]. Nasal NK/T cell lymphoma refers to malignancies originating in the nasopharyngeal area, causing destruction of the nasal cavity, paranasal sinuses, orbits, and surrounding structures. Common presenting signs are midline mass with obstruction, nasal bleeding, and perforation of the hard palate [3]. This subtype is more common in Asia and South America and relatively rare in Western countries [2]. The average age of affected patients is 50-60 years old, and it occurs three times more in males than females [3]. Due to its rare incidence and deceiving presentation, it is often misdiagnosed as chronic sinusitis, granuloma, or fungal infection; thus, the final diagnosis and treatments can be delayed.

Pars plana vitrectomy and internal limiting membrane peeling (ILM) is the standard of management for full thickness macular hole. After the introduction of ILM peeling, the success rate of surgical management of macular hole has improved tremendously. Several studies have shown that internal limiting membrane (ILM) peeling in macular hole surgery causes changes in the macular thickness and decrease in retinal sensitivity [1-10].
Advances in optical coherence tomography (OCT) technology over the years have made possible to assess the macular morphology and thickness more accurately. It has also made possible to study the structures in the layer of the retina more accurately. With this objective, a study of macular changes after brilliant blue assisted ILM peeling for idiopathic macular hole was done using optical coherence tomography (OCT).

For a psychological test or instrument to function properly as intended, items in the test should measure respondents’ performance fairly across different groups of respondents such as male and female. In psychometric literature, the concept of differential item functioning (DIF) has been introduced to address the differential group performance on an item when the groups are equated at the same level of ability or latent trait status. This article introduces the concept of DIF while making a clear distinction of DIF from item bias and simple group performance difference since the civil rights error of the 1960’s in the United States, inequity has become a critical social issue. The area of educational and psychological testing is no exception. The use of testing as a sorting mechanism [1] has brought equity concerns to many people, specifically the testing enterprise.

Defined, agreed upon and even legislated in the field of primary care, why is minor surgery not a usual practice in the first level of health care? [1] We will talk about "rescuing” a family doctor's own practice and coordinating it with specialized attention to work as a team, without forgetting the important participation of nursing, looking for the quality of care and the improvement of work activity together with the satisfaction of both the user and of the professional.
Continuous training, adequate infrastructure and motivation of professionals are key. Finally, patient-medical motivation and satisfaction do not justify minor surgery on their own in primary care, but together with a precise surgical indication, safe and necessary practice, coordination with specialized care, plus scientific endorsement of what has been done and published. Until now, in addition to administrative support, they are real variables for its development and implementation [2]. Perhaps the medical super-specialization and the overloaded work schedule are two of the many reasons that have led to the practice of minor surgery in primary care reduced to simple sutures of injuries by nursing in most cases ... Beta-blockers are commonly used to treat tachyarrhythmia's in thyroid storm. However, there have been several cases in which cardiovascularcollapse has been reported after administration of Propranolol. This phenomenon seemed to be more common in Chinese. The article presented an unusual case. A 14-year-old girl with thyrotoxicosis presented with wide complex tachycardia in an electrocardiogram (ECG). The rhythm was converted to a trial tachycardia after cardio version. Propranolol was started to control atrial tachycardia. The girl developed cardiorespiratory arrest soon after taking Propranolol. She required high-dose inotropic support and the use of an intra-aortic balloon pump after resuscitation

Objective: The first cause of death worldwide is cardiovascular disease (CVD). CVD covers a wide array of disorders, including diseases of the cardiac muscle and of the vascular system supplying the heart, brain, and other vital organs. This research aims to the comprehensive impact that a series of lifestyle data from a population has on the main cardiovascular risk factors. Such predictions of the influence of lifestyle on cardiovascular risk factors could be useful.

Material and methods: A cross-sectional study was designed; the subjects who lived in Minodar, Iran were interviewed by trained nurses using a structural questionnaire. Data were processed from a sample of 393 subjects of both sexes aged 26-79 years old. The output data were: high/low cholesterolemia, HDL-C cholesterol, triglyceridemia. The input data were: sex, age, build, weight, marital status, Individual's status in the family, physical activity, hours of sleep per day, smoking, tobacco type, BMI. Two predictor models including artificial neural network and linear regression were applied
