Health Related Quality of Life and Wellness of Aging LGBT Populations in Wilmington and the Cape Fear Coast by Noell L Rowan in Geriatrics Studies

Estimates consistently state that by 2030, 20% of the U.S. population will be age 65 and older, a total of 70 million elders [1,2]. Within this group, older LGBT persons are predicted to number 1.75 to 3.5 million, "with approximately 500,000 gay men and lesbians turning age 50 each year" [1,3]. Goldsen & Kim [4] report an estimate of more than 2.4 million adults in the United States ages 50 and older identify as lesbian, gay, bisexual, or transgender (LGBT). Cultural ageism and heterosexism, along with fear of discrimination at the hands of health care providers in inpatient and outpatient, short-term as well as long-term care settings, can and often do have a direct negative effect on the quality of life of older LGBT persons [1,3,5].

In engineering, reliability is defined as the ability of a device to perform the preset function for the given time under the given conditions. The foundations of mathematical theory of reliability were laid in the 1950s due to the needs of aeronautic machinery, electronics, problems of communication and management, etc. [13]. Like in engineering, each and all biological systems are devices constructed to perform the preset functions according to the information planes, i.e. - the genetic programs. Biological constructs perform their functions in the presence of a great number of random factors which disturb all functional strata, starting from the molecular level of organization and to ecosystems inclusive. Therefore, similarly to technical devices, biological constructs are not perfectly reliable in operation, i.e. - for each and every device normal acts of operations alternate with stochastic (random) malfunctions or failures.

Israel, as many other countries, must cope with issues that arise from demographic changes that are expressed both in the size and composition of the population. One of the prominent changes is in the age-structure of the society, which is especially influenced by the ageing of the population. Israel's population consists of 12.8% Jewish elders and 4.4% Arab elders. However, the proportion of disabled Jewish elders is 16% compared with 31% among Arab elders [1]. Disability leads to loss of independence, increases consumption of health services, and raises the cost of medical treatments. Among minorities, lower rates in Health Services use were found, since cultural differences are often not considered. Lack of sensitivity towards minority needs may lead to a gap between potential consumers and existing services [2-6].

Role of Pharmacists in Improving Drug Treatments - Crimson Publishers

 


Problems in pharmacotherapy in elderly patientscan derive in an increase of drug adverse effects, hospitalizations and length of stay, morbidity and mortality. There is a great awareness of this problem and much workhas been done in order to reduce the polypharmacy and improve the quality of the drug prescription in these patients. With this aim, we have different tools and initiatives, being the STOP-START and Beers Criteria those upstanding. However, it is not easy to implement those criteria or to do perform other measures when confronted with complex treatments. Comorbidityand constant worsening of the pathologies in these patients makes very difficult to reduce in the long runthe number of drugs that they have to take. Moreover, new drugs for pathologies that previously had not pharmacologic treatment are constantly marketed.
