Transvenous Endovascular Embolization of Ruptured AVM, Two Cases Report: Techniques in Neurosurgery & Neurology: Crimson Publishers_Journal of Neurology Neurosurgery and Psychiatry


Cerebral arteriovenous malformations (AVMs) are the most common congenital cerebrovascular diseases with high risk of intracranial hemorrhage (2% annually) and mortality (1-1.5%). Currently, there are three directions in the treatment of this disease: endovascular embolization, microsurgical removal, and radio surgical therapy. We present two cases of AVM treated by endovascular transvenous approach.

Brain metastases represent a critical stage of oncological disease and its frequency has increased over the recent years. The treatment of brain metastases has moved from a conservative approach to an active management that should be individualized for each patient. In cases of single brain metastasis, surgery or radiosurgery should be considered as first option for treatment. In cases of multiple lesions, whole-brain radiotherapy is the standard of care. The aim of this review is to present general aspects including new approaches in the management of patients with brain metastases.

Depression is one of the most common serious medical and psychiatric disorders, with greater than 200 million adults worldwide suffering from major depressive disorder (MDD) at any given time, according to a 2003 report by the World Health Organization titled Investing in Mental Health. According to the U.S. National Comorbidity Survey Replication published in 2007, more than 16 million adults in the United States, which represents approximately 6.7% of itsentire adult population, will suffer from MDD in a 12-month period. The quest to come up with a superior antidepressant is in full effect among biotech and pharmaceutical companies. Although there are some companies that are still tweaking various formulations of serotonergic, noradrenergic, and dopaminergic substances i.e. triple reuptake inhibitors others are thinking outside the box.

Desmoplastic non-infantile ganglioglioma (DNIG) is a rare tumor entity and only a small number of cases have been published until now in the literature. Although its infantile counterpart (desmoplastic infantile ganglioglioma [DIG]) has been already recognized and included together with desmoplastic infantile astrocytoma (DIA) in the WHO Classification of CNS tumors, DNIG has not been accepted as a distinct tumor entity. The reason for this is obviously the scarcity of the neoplasm and subsequently the absence of substantial information regarding its pathogenesis and its association to the infantile form. Herein we present an interesting case with an unremarkable clinical presentation, which came to medical attention after the performance of imaging investigations, for staging purposes of a multifocal papillary thyroid carcinoma. Although the tumor showed the typical biphasic nature, consisting of a solid and a cystic component, its localization in the left ventricular trigonum, was indeed uncommon. Finally, both the histomorphological and the immunohistochemical findings were consistent with the diagnosis of a desmoplastic non-infantile gangioglioma. The occurrence of DNIG with the various clinical manifestations as well as the diverse imaging features, underlines the need of histopathological examination, in order to establish the final diagnosis.

Elsberg syndrome (ES) is an infectious syndrome consisting of acute bilateral lumbosacral radiculitis, often accompanied by myelitis confined to the lower spinal cord, and is frequently a manifestation of reactivation, or occasionally, primary herpes simplex virus 2 infection [1]. The onset of ES is consistently acute and symptoms includes: Urinary retention, urinary incontinence, bowel incontinence, saddle anesthesia, exaggerated/pathologic lower limb tendon reflexes or absent lower limb tendon reflexes, Loss of limb sensation and Leg weakness [2].

Intracerebral hemorrhage (ICH) accounts for 8-13% of all strokes and is the most frequent fatal form of stroke and has the highest level of morbidity of any stroke subtype. For patients with both intracerebral hemorrhage and intraventricular hemorrhage (IVH), expected mortality is 50-80% and good functional outcomes in less than 20%. A common question for both Neurologists and Neurosurgeons is how to manage patients with ICH less than 30 ml with large intraventricular hemmorhage with obstruction of the third and fourth ventricle.
