Abstract

Background: Intramedullary dermoid cyst is a type of rare congenital intraspinal lesions especially those located in the conus medullaris and not associated with spinal dysraphism. The Management of DC is especially difficult in eloquent areas such as the conus medullaris, with tendency to recurrence and poorly respond to oncologic treatment.

Objective: We aim to provide a summary of the pathology, clinical presentation, and management strategies of dermoid cysts involving the conus medullaris with review of the literature.

Case presentation: A 37-year-old woman presented with progressive low back pain and loss of bowel and bladder function. His magnetic resonance imaging (MRI) scan showed a heterogeneous tumor with tethering of the spinal cord. He was taken for a midline myelotomy that drained yellowish keratinous fluid and decompressed the cyst. No aggressive attempt at complete resection of the cyst wall was undertaken. She made a complete recovery after surgery with complete resolution of her symptoms.

Conclusion: Dermoid cyst of the conus medullaris without any congenital spinal dysraphism make this case unique. Surgical control and restraint are the key for optimal management and good recovery

Abstract

In the hydrodynamic formulation, the problem of liquid filtration from a channel filled with water through a soil layer with an underlying pressure horizon of relatively high permeability in the presence of evaporation from the free surface of groundwater is solved. When considering flows from channels, it is usually assumed [1-5]. that filtration occurs only through their bottom, which is usually taken as a horizontal segment. Taking into account the influence of the depth of water in the channels, that is, the study of movement not only through the bottom, but also through the slopes of the channels makes an additional angular special point in the physical area, which significantly complicates the solution of the problem. In this paper, the method developed earlier [6] is used to study the regime of ground water when filtering from such channels filled with water, in the presence of evaporation from a free surface.

In the framework of the theory of plane steady filtration of an incompressible fluid according to Darcy's law deals with the flow of rectangular channel of width 2l with water depth H in the soil capacity T, underlain by the well permeable pressurized horizon is relatively high permeability, the pressure of which is equal to H0 (0<H0<T), in the presence of a uniform intensity of evaporation ε (0<ε<1) with a free surface. To study the flow formulates a combined multi-parameter boundary problem of the theory of analytic functions which is solved using the method of Polubarinova Kochina PY [1-5], based on the application of the analytic theory of linear differential equations of Fuchs class [7] and the method of integrating such equations with four regular singularities [8-10], which is characteristic for problems of underground hydromechanics [11-15]. It should be noted that taking into account the specific features of the movements under consideration allows you to get solutions to problems in a closed form through elementary functions, which makes its use the most simple, convenient and effective

Abstract
Capnography is a simple non-invasive monitoring technique that provides prompt and valuable information about patient’s ventilation, perfusion and metabolism. We are presenting a case of a 10 year-old boy with a history of repaired Tetralogy of Fallot (TOF) undergoing a cardiac catheterization for recurrent left pulmonary artery (LPA) stenosis. A variable plateau capnogram waverform was detected and attributed to ventilation-perfusion abnormalities from differential lung perfusion secondary to critical LPA stenosis. Capnogram can prove vital monitoring tool in detecting critical perfusion abnormalities

Introduction
End tidal carbon dioxide (EtCO2) monitoring using capnography is a standard of care in anesthesia and essential for patient safety [1]. Intraoperative capnography provides information about CO2 production, pulmonary perfusion, alveolar ventilation and respiratory patterns. The different capnogram waveform shapes can reflect abnormalities that can be simple and common or can be a sign of imminent life-threatening complications [2,3]. Here we present a case of an unusual capnogram waveform that was detected during a diagnostic and therapeutic cardiac catheterization in a pediatric patient with history of Tetralogy of Fallot (TOF) repair with transannular patch and left pulmonary artery (LPA) patch.

Abstract

Objective: To determine if there is a difference between uterine incision techniques (vertical vs transverse) in terms of clinical results.

Methods: All women with leiomyomas who underwent open abdominal myomectomy (n=61) between March and August 2016 in the Clinic of Gynecology and Obstetrics, Zekai Tahir Burak Women’s Health Care Training and Research Hospital were included and the clinical results were reviewed prospectively.

Result: The estimated blood loss during myomectomy increased in transverse group compared to vertical group (809, 33±483, 34ml vs 405, 32±180, 95ml, p<0.001). The median operative time is 60 minutes and the patients are discharged second day after surgery. No intergroup statistically differences are observed in surgical procedure.

Conclusion: Open abdominal myomectomy causes blood loss however, in transverse incision there is a higher level of estimated blood loss compared to vertical group

Abstract

Ocean acidification leads to a wide variety of responses from tropical coastal ecosystems. Coral reefs are most vulnerable with most coral species exhibiting declining calcification rates with decreasing pH and carbonate chemistry parameters. Some corals show resilience to acidification likely due to active physiological regulation of their calcifying fluid. Other calcifying organisms, such as some foraminifera and coccolithophores, exhibit negative responses, whereas some symbiont-bearing calcifiers respond positively, to increasing acidification. Seagrasses and brown macroalgae thrive under acidified conditions, with increasing rates of primary productivity. Some tropical coastal fish species are resilient, and in some species, respond positively, to acidification. Some tropical species show complex, nonlinear responses to declining pH and carbonate chemistry. Factors that influence the ability of a species to adapt to and/or resist acidification include food supply, nutrient availability, temperature, diet, interactions with symbionts and other organisms and species and community diversity. Interactive effects of ocean acidification with other climate change parameters, such as elevated temperature, play an important but poorly understood role in determining the resilience and vulnerability of tropical coastal species, communities and ecosystems. Some short-lived species can undergo acclimation and/or adaptive evolution to increase fitness in the face of acidification. Biota living in tropical estuarine and nearshore environments, such as mangroves, seagrasses and intertidal and subtidal inshore benthos, are unlikely to be significantly affected by future acidification as such environments exhibit very wide variations in water and sediment pH and carbonate chemistry

Abstract
Recent publications have reported high prevalence of Candida parapsilosis Sensu Stricto in oral cavity niches. Our research group performed a pilot study in 2017 which showed that of the psilosis complex, Candida parapsilosis Sensu Stricto is the species most frequently isolated from oral cavity niches. Under inflammatory conditions, the probability of recovering it is almost four times higher, and it displays higher biofilm-forming capacity in-vitro, differing significantly from isolates of the same species obtained in conditions of eubiosis. This led us to hypothesize that an oral environment in dysbiosis overregulates virulent genes, promoting a more pathogenic phenotype. Accordingly, we decided to re-evaluate the in-vitro biofilm formation assay using a larger sample, under two nutritional conditions, using colorimetric methods for quantification. In addition, the results obtained were validated by imaging techniques. For both clinical conditions (eubiosis and dysbiosis), high biofilm forming phenotype predominated with both reading methods, and for both nutritional conditions tested. XTT showed significant difference between absorbance values for isolates from buccal dysbiosis and eubiosis (P=0.0025). Candida parapsilosis Sensu Stricto cells that colonize oral cavity niches are basically strong biofilm formers, regardless of in vitro growing conditions. However, the oral cavity in dysbiosis probably promotes virulence in this species due to inheritable epigenetic modifications

Air Pollution, reactive oxygen species damage, and genetic or epigenetics abnormalities may be responsible for some cases of male infertility. On the one hand, the occurrence of female infertility is related to the change of life style, the decline of ovarian reserve and egg quality caused by the later reproductive age of women, on the other hand, it is also related to the deterioration of living environment

Abstract

Background: The severity of coronary artery disease (CAD) is directly related to the quality of glucose control in diabetic patient. Additionally, diabetes is associated with increased mortality following acute myocardial infarction compared to general population.

Objectives: To evaluate the association of HbA1c level and severity of CAD, and outcome of non-diabetic patient with STEMI in our hospital.

Patients and methods: 60 consecutives non-diabetic patient with acute ST elevation myocardial infarction were treated with thrombolytic therapy included in the present prospective study. Blood glucose and HbA1c level of all patients were measured within 3 hours of admission. Patient were divided into 3 groups according to HbA1c level: with cut-off 6.5% as diagnostic criteria of diabetes mellitus according to (American diabetes association) group (1) 6.5%, group (2) 6.5 to 8.5%, group (3) 8.5% and above, in hospital. Mortality and morbidities of acute STEMI were compared between groups.

Result: The mean age was 63±15 year and mean body mass index was 26.6±6 kg/m², 24 patients (40%) had history of hypertension, 27 patients (45%) of dyslipidemia, 36 patients (60%) were smoker. We found 45 patients with HbA1c≤6.5%, 5 patients with HbA1c 6.5-8.5%, 10 patients with HbA1c≥8.5%. There was strong correlation between admission of HbA1c and admission glucose level (P<0.001). Infarct size as measured by peak creatinine kinase, was not correlated with HbA1c level.

Conclusion: HbA1c is an important risk marker in the absence of history of diabetes mellitus in patients with AMI. The optimal management in these patients may contribute in decrease hospital mortality
